Delaware 2-1-1 is a committed advocate and participant in strategic discussions to develop solutions that reduce or prevent homelessness. We will send referrals to all service providers, including those operating outside of the Continuum of Care network, who can support our homeless citizens. It has become very apparent that the challenges identified in Delaware 2-1-1 Pilot Report & Recommendations: Centralized Intake (dated 10/29/2014) cannot all be resolved immediately to help people who are homeless this winter. Therefore, the purpose of this petition is to identify and agree on those "vital few" corrective action steps we can implement within the next 15 days or less. All who sign this document agree and support the actions outlined below.

For homeless calls received, Delaware 2-1-1 will:

1. Continue to act as the first number to call to assess caller’s need(s), identify and provide resources (e.g. financial assistance to delay utility shut-off) to keep individual or family housed or meet other needs (e.g. food)

2. Complete the full standard assessment and process to effectively document information on those who are immediately homeless into CMIS

3. Return to guiding Delawareans who are immediately homeless to their closest service provider(s). Prior to terminating the call, we will confirm bed availability for placement with the provider

4. Share targeted information and data with key stakeholders - which includes but may not be limited to State Service Centers, Service Providers, Homeless Planning Council of Delaware, Delaware Interagency Council on Homelessness (DICH), CoC Network or other governing bodies as needed

Service Providers, who make available emergency, temporary, transitional, supportive or permanent housing for the homeless, will:

1. Accept and process calls from Delawareans who are immediately homeless from Delaware 2-1-1

2. Access client records (if documented by DE211) or create new client records for all homeless Delawareans looking for shelter or housing programs in CMIS

3. Confirm bed and housing placement with Homeless Planning Council of Delaware (HPCD) in CMIS within 30-minutes of client arriving at agency

Homeless Planning Council of Delaware will:

1. Manage data and provide routine reports on the incident of homelessness in Delawareto participating agencies or external stakeholders who participate in the coordinated assessment process

2. Collect and disseminate eligibility requirements for bed and housing programs to support CoC and other funding streams documented in CMIS

3. Make necessary improvements to CMIS system that would enable Delaware 2-1-1 to complete full assessment and provide access or "permission" for targeted Service Providers to "view" call records entered into CMIS by Delaware 2-1-1

4. Develop an effective strategy to prioritize bed or housing placements statewide

5. Create standard application form and protocols to effectively transition homeless individuals and families from shelters to rapid rehousing and permanent supportive housing programs
