Dear Patient,

We are greatly disappointed to have been notified by Health Partners, your health insurance provider, that they intend to terminate our vein treatment coverage agreement with them effective November 25, 2017. Health Partners did not provide their reason for this termination. You may have already received notice of this directly from Health Partners.

USA Vein Clinics provide a unique, important, and valuable medical service to patients like you in the Philadelphia metro area, who rely on Medicaid for their phlebology services. There are many residents who will no longer have access to our services and care, provided by our top vascular specialists.

Along with our protests, many patients similarly affected are signing the attached petition with the goal of asking Health Partners to reconsider their unfounded and inexplicable decision. This is one of several efforts being undertaken to prevent interruption to your medical care.

We hope you will join us by signing and returning the attached petition requesting Health Partners to allow our practice to remain as a contracted Health Partners provider. We appreciate your support and hope to be allowed to continue to provide you with the best in vascular care. We will notify you of Health Partners’ response when it becomes available.

We the undersigned patients of USA Vein Clinics of Philadelphia LLC rely upon the phlebology services that we receive from this practice. This phlebology practice is located within a reasonable vicinity to which we feel comfortable commuting. We have come to rely upon the physicians of this practice and want to continue to receive our phlebology services from this practice and its physicians. We are happy with the services received from this practice and its physicians and do not want to switch to other practices unknown to us.

To that end, we hereby demand that Health Partners allow USA Vein Clinics of Philadelphia LLC to remain as provider. This is a matter of upmost importance to us, we demand that Health Partners listen to us as its customers, and accommodate our above request.
