We the community & Robert Lee need some help with getting signatures for the City of Lubbock. Please feel free to read below and sign in confidence.

By signing this agreement, and in being allowed to participate in voluntary

installation, maintenance, upkeep, and cleanup activities at the City of

Lubbock Disc Golf Courses at Mackenzie, Mae Simmons, McAllister

Parks, I _____________________do hereby waive, release, and forever discharge the City of Lubbock and its employees from any and all responsibilities of liabilities for injuries or damage resulting from my participation in any activity at the disc golf courses. I also agree to abide by the rules, policies, and procedures of the disc golf courses and equipment.

I understand that disc golf activities involve a risk of injury and even death,

And that I am a voluntarily participating in the activities and using the equipment and machinery with knowledge of the dangers it involves. I

assume any and all responsibility for and agree to pay all claims, expenses, and medical bills, chiropractic bills, subrogation claims, and any and all claims incurred as a result of the use of said disc golf facilities.

I warrant that this Waiver and Release of all claims has been read and fully

understood. This Waiver and Release of all claims and covers all claims known and unknown, present or future, resulting from voluntary use and the activities of the disc golf facilities by me. I am signing the Waiver and release of my own free will and accord without any reliance.

I HEREBY AGREE TO EXPRESSLY ASSUME AND ACCEPT ANY AND ALL

RESPONSIBILITY OF RISK OF INJURY.
