A number of Tesla Motors Model S drivers have reported unintended acceleration events in owner forums. These generally appear to occur when the brake is depressed, but the foot is also partially on the accelerator.
This issue is generally addressed by other auto manufacturers by implementing "Brake/Throttle Override". Roughly speaking, application of the brakes should override or disable the throttle, preventing unintended acceleration during a panic braking situation, or when a physical obstruction such as a floor mat traps the accelerator pedal.
