Due to his abrasive nature, threatening tactics, and bully like demeanor, we feel Frank Cook should be removed as the head of medical records at Jupiter Medical Center.

Understanding that medical record documentation is an important part of today's medical field, more reasonable, constructive and user friendly tactics should be implemented to advance the hospital's goals.
