We the undersigned hereby affirm the reinstatement of Angela Weaver, Rn as Director of Nursing for Life Care Center of Orange Park effectively immediately. The Associates of Life Care of Orange Park have recongnize the great improvement in our facility due to Angela Weaver's hard work and dedication. It would be a step backwards for the facility and nursing department with her departure.
