1. There needs to be a second check person to avoid medication errors.
2. Indication and diagnosis is important to avoid counter reaction of medication.
3. Proper control is compulsory to avoid misuse of medicine.
4. Pharmacist are specially trained about medicines whereas doctors have many other things to be concerned about. As it is, now itself concentrated drugs are been given instead of diluted concentrations. Even they are humans, not robots. By doing this, you are increasing their workload and this is going to cause more errors in prescriptions and the patients are going to be seriously affected.
6. With this, patients need to be more educated on their drugs to avoid errors.
