Dear Teamsters/Local 135 & RAH Payroll Dept:

Effective upon submission of this petition to the proper authorities, I am requesting that my dues authorization check off be stopped. I no longer want my dues to be removed from my paychecks, I will mail my union dues myself as this is my right under Article 20 B. I'm doing this in an effort to show my unity with my co workers and the fact that we aren't going to stand for anymore business as usual.
