Open Letter to the DSM-5

I am a teacher and also hold a Diploma of Counseling. I am greatly disturbed by the increasing influence of drug companies in the backgrounds of so many in positions of authority in these so called diagnostic areas. The huge amount of lawsuit fines for off line marketing and failure to disclose side efects must call into question the medications promoted with the increasing number of new' 'disorders'.

I have friends who are extremely high up in psychiatry who are far from happy with many of the diagnostic criteria they are expected to use . Their personal opinions about psychiatry are that it is far from therapeutic for " patients " and harm is done instead . They believe the system should be more user friendly and that over diagnosis is damaging people who would in most instances fair better if they spoke to friends or relatives about their troubles . There is no definitive clinical pathology for the vast majority of mental illness and because of this people will undoubtedly be exploited . Your letter states the obvious !

Psychiatry is the only profession or branch of medicine that prescribes neurotoxic medicine and controlled substances without a physical exam which in all other forms of medicine controlled by state medical boards require physicians to exam their patients doing a head to toe exam looking for possible reasons for ailments. .Emotional distress cannot be contained in an physical exam, therefore many if not all of the DSM-V disorders are virtually reasons to medicate and more and safer alternatives need to be explored as they should be without resorting to pharmaceuticals which can shorten lives dramatically.....

The potential for epidemiological trend diagnosis in clinical practice, as well as the substantial increase in off-label psychiatric prescribing in pediatric cases seems like an afterthought in light of DSM 5 revisions. The DSM changes will have real implications "on ground" in clinics and in our practices. Will we never have the courage to say no to the economic pressures of the for-profit pharm industry and to learn from our arrogant, damage ridden past?

I have for years worked with young adults who come my clinic with a handful of diagnoses and two handfuls of medicines. Many leave without symptoms of with symtomes they manage without medicine when we see the person and the suffering behind the symptoms and not only the symptoms.

DSM V continues the trend of dropping, redefining and replacing conditions. Whilst I don't think a positivist approach is entirely wrong, the history of this text illustrates, embarssingly the errors of a positvist approach. Too often I have heard course leaders insist that this is the text to instruct trainees how to understand mental health conditions, I assume they have no understanding of the controversy this book generates.

As both a professional lecturer and mental health counsellor, I would be most happy to organise a workshop for the DSM committee to elucidate the difference between disease and illness. Such crucial basics seem to be very unfortunately omitted from the most curricula in medical education.

The fact that a child can be given drugs because he or she isn't very good at maths just screams of how wrong the DSM is.

No one seems to be talking about this but the PTSD dx needs reformed. Criteria A now includes vicarious traumatization and complex bereavement. These are not PTSD.
