Hello all,



I am making this petition on behalf of my close friend and coworker Chris Hlavaty. Chris tragically passed on 6/10/24 at approximately 10:45pm riding his motorcycle by a hit and run driver who left the scene of the accident. Chris was a member of the Hagerman Fire Department in East Patchogue, NY and worked full time as a Motor Vehicle Operator at Emergency Ambulance Service based out of our Suffolk Base in Bohemia, NY. He had a bright future which was tragically cut short at the age of 22 and had aspirations of becoming an EMT set to graduate from EMT school in August 2024. He was someone that always knew how to brighten up any room with his jokes and always knew how to make everyone laugh and put a smile on his face. He was a son to an amazing family, an amazing boyfriend to his girlfriend Kirsten, an amazing friend to many, a fellow brother in the fire service with his brothers and sisters from the hagerman fire department, and an amazing coworker to all of his colleagues at Emergency Ambulance Service. I am petitioning to have the ambulance that he used most days that he was on shift Ambulance 213 be dedicated in his honor and will always remind everyone of how dedicated and caring he was for his coworkers, patients and the community. Love ya brother your gone but you will never be forgotten❤️


Update: Hi everyone! On behalf of Chris’s family, friends, the Hagerman Fire Department and his colleagues at EAS I want to thank everyone for the amount of support that this petition has gotten through the comments of remembrance and all the signatures recieved our voices have been heard. The idea was brought to the attention of my bosses and has been approved. As of today mourning bands have been placed on the back of 213. Chris if your watching this brother remember we all love ya and you will never be forgotten brother Rest in Peace❤️

I met with the leadership team at the base this morning to discuss the plans for unit 213. They are listening to our request because of how clearly you have shown that Chris deserves this memorial. I will keep pushing until we see his name on that truck.

Seeing one hundred people come together to honor Chris truly touches my heart. His laugh and his spirit deserve to be remembered on the side of that truck every single day. Thank you for helping me keep his memory alive.

I knew Chris when he was a student at SHS- a great kid who obviously liked to help others. A fitting tribute would be to name the ambulance in his memory. May he RIp

Chris was a great guy, and always appreciative of everything, especially my silly little pictures I’d take of him at fire calls. I’m very happy to have known him. My sincere condolences go out to his departments and his family.
