We, the undersigned, do hereby petition the United States Food and Drug Administration (hereinafter, "the FDA") to immediately and without delay cease and desist from further patient-unauthorized electronic, paper, telephonic, facsimile, or by any other means, gathering chronic pain management patient and any other type patient personal and private medical data; to immediately and without delay cease and desist storing, using, disseminating, sharing, publishing, selling, or in any other way violating the doctor-patient privilege and in any other way violating the 4th Amendment rights under the Constitution of the United States of said patients; to immediately and without delay cease and desist requiring physicians, therapists, nurses, radiologists, and any other health care professional to submit any private and personally-identifying patient data; to immediately and without delay redact all patient personally-identifying data from such records and data as are already in the possession of the FDA, and to furnish each patient so affected verifiable written proof that said redaction has been performed; to return to each patient so affected each, every, and all copies of his or her medical records as the FDA currently has in its possession; to publicly apologize to the chronic pain management and any other patient group or segment which has been so violated without just cause, proof of need, subpoena, or individual patient-case court order; and to change such internal policies and rules as required to satisfy the demands of those American people affected by the FDA's autocratic and unauthorized data gathering practices.
This petition is hereby submitted pursuant to the 1st Amendment to the Constitution of the United States, specifically, to wit, the petition clause of said Amendment, preserving the People's right to petition the government for redress of grievances.

Specifically the petitioners' grievance is that the FDA, without signed authorization of the undersigned (and any and all other so-affected American people), has engaged and is engaging in the illicit gathering of patient personal and private medical and other data and records without our informed consent and specific, limited authorization.
