I, the undersigned, have had the Belmont Abbey College Athletics Sport Related Concussion Management Plan explained to me and I understand its contents and my responsibilities within it. I have been educated on what a concussion is and how it will be managed at Belmont Abbey College. I have also had the opportunity to ask any questions that I may have about this plan.
