To whom it may concern, This is concerning the matter of terminating any and all legal parental rights to William Noah Temple (minor child). Angela Marie Temple is willingly excluding herself from any and all rights concerning said minor child, Which includes but not limited to, any medical decisions, educational decisions and money benefits. This arrangement being permanent and final.
