    Comments: The RUC is biased and the Gov't CMS is colluding with the AMA in increasing the dependence on forced non private contractor like systems be they forced fees and devaluation of primary care and other office visit cognitive based medicine. The present coding system is just as offensive and equally plays a role in distorting and destroying pay and quality of care especially in fields that live or die by the office visit with little or no expensive procedures to balance out the pitiful payment for all the coginitive doctors really do.

    Comments: I agree wholeheartedly with the above summary with the RUC and it really is why the costs have skyrocketed, the quality and access declined and why Primary Care as a speciality(FP) will cease to be soon.

    Comments: The RUC is counter to the goals and ambitions for saving the country's healthcare system. It rewards proceduralism, utilization, and devalues cognitive work, patient engagement efforts, and disease management services. It is clear to me that the latter is where our efforts should be directed. The RUC system is philosophically antagonistic to this vision and needs to be abolished to allow for a more equitable and innovative method of reimbursement to flourish. I am a fourth generation family physician, providing care to the same community as my great grandfather. Our family has provided comprehensive care for the last 115 years to my rural hometown. Unfortunately, I have witnessed the sad atrophy of today's primary care physician from the proud tradition it embodied when I was a child. It is clear to me that the nidus of this decline can be directly attributed to the date the RUC was implemented. It is ABSOLUTELY no surprise to me that we are witnessing a shortage of docs in rural communities.

    Comments: I have been practicing medicine as a primary care provider since 1987. The RUC has damaged my profession by reducing incentive for physicians to practice primary care. We are seeing a national exudus of Family Practice, Internal Medicine and other primary care toward procedure oriented medical care. This does nothing but drive up the cost of medicine. Primary care providers are in the best position to hold health care cost down while provide the best in outcome based therapies.

    Comments: It's time the AAFP told the truth and acted upon principle. The RUC should be abandoned. It would probably be better if CMS itself studied the inputs, stresses, etc for various specialty care episodes and figured this out. But abandoning the consult codes was a sloppy approach. A much more studied and comprehensive approach should be taken, considering length of training, work stress, time, etc. The entire RBRVS needs to be re-valued.

    Comments: Please consider the costs of business when determining pay scales. If I knew about the RUC, I would never have become a family doctor. Even as doctors allign in ACOs, the specialists get more from hospital administrations. We are completely overwhelmed with paperwork and red tape. The administrative hassles are ridiculous. I do more and more paperwork every year. I am burning out. Please support primary care.
