It rewards proceduralism, utilization, and devalues cognitive work, patient engagement efforts, and disease management services. It is clear to me that the latter is where our efforts should be directed. The RUC system is philosophically antagonistic to this vision and needs to be abolished to allow for a more equitable and innovative method of reimbursement to flourish. I am a fourth generation family physician, providing care to the same community as my great grandfather. Our family has provided comprehensive care for the last 115 years to my rural hometown. Unfortunately, I have witnessed the sad atrophy of today's primary care physician from the proud tradition it embodied when I was a child. It is clear to me that the nidus of this decline can be directly attributed to the date the RUC was implemented. The RUC has damaged my profession by reducing incentive for physicians to practice primary care. We are seeing a national exudus of Family Practice, Internal Medicine and other primary care toward procedure oriented medical care. This does nothing but drive up the cost of medicine. It would probably be better if CMS itself studied the inputs, stresses, etc for various specialty care episodes and figured this out. But abandoning the consult codes was a sloppy approach.
