By signing this petition, I am requesting the purchase of an Olympic Bar Rack, Olympic Bars, and a Squat Cage for use in the Varsity Gym located at 425 East Washington Street, Ann Arbor, Mi. 48104 from EDR Management.

I affirm that by signing this petition that I am a resident of the Varsity Apartment at the address stated above.

I will safely use the requested Olympic Bars and a Squat Cage in the weight room and will only use the equipment under the supervision of a spotter.

I affirm if this equipment is purchased, I wave any liability from injuries due to these machines without the use of a spotter.

All things considered, I understand that by signing this petition, there is no guarantee that EDR Management will fulfill this request, however that my signature confirms my agreement with the policies stated above.
