1. Even though my request for disability was denied, I, per Title 20, Chapter III, Part 404 (Subpart J) and Part 416 (Subpart N) I have submitted a Request for Review of Hearing Decision/Order within the alotted time limit of 60 days from date of Judges decision which was February 21, 2012 (See EXIBIT "A") 2. My disability which is Degenerative Disk Disease and Osteoarthritis in my lower back originally allowed me to work a limited schedule. However, over the last month the Disease has now moved to my neck (See EXIBIT "B") which is causing me not only pain but a drastic reduction in use of my right arm, and at this time have problems holding even a cup of water. 3. If my petition for disability is awarded my monthly income will be a gross amount of One Thousand Four Hundred and Eighty-Nine Dollars per month. 4. In 2010 my gross income (See EXIBIT "C") was $19,630 with a refund of $264.00 was kept by the IRS due to back taxes owed totaling $49,000.00 with interest from 2005. 5. In 2009 my gross income (See EXHIBIT "D") was $4,391. with a refund of $816.00 kept by the IRS also due to the back taxes owed with interest since 2005. 6. From February 12, 2006 through May 12 2009 I was incarcerated earning no income. 7. Having been unemployed since February 21, 2011, and unable to find employement due to the factors of a Feloney conviction, and my Disability which restricts me from doing many types of work I started my own bookkeeping firm, studied passing the comententcy exam with the IRS and received my PTIN number allowing me to submit tax returns to the IRS, and state returns. However, due to the worsing of my neck and arm even typing has become very diffacult. Presently I have I have 4 clients that pay a monthly bookkeeping fee of $806.00 per month, and no other sorce of income. If my condition worsens then i will be without any income.
