I am setting up a campaign to change the way people with long term conditions. And am hoping to get these changes into place all over the England. I would like to make sure that everyone with long term conditions are entilted to free prescriptions,make sure that housebound patients get home care by doctors despite thier age or what is wrong with them, to let patients have control of thier treatment and more say about thier care and get treated better by all health professionals. Also I would like to make sure that all disabled and people with long term conditions get assesments and equipment within 2 weeks of equiring.
