To: Nancy Adams, Chair of the Library Faculty Organization; Erin Burns, Vice-Chair/Chair-Elect of LFO; and other Members of LFO Council:

We, the undersigned faculty of Penn State University Libraries, and customers of Highmark Blue Shield, express concern about on-campus Biometric Screenings, online Wellness Profiles, the usage of WebMD and ICH to collect and store personal health information, the $100 monthly noncompliance penalty, and other requirements of Highmark's "Take Care of Your Health" initiative.

Therefore, we request:

*That LFO hold an "open mic" forum, at a place and time accessible to all University Libraries employees (including staff), and with the Libraries Administration, Human Resources representatives, and Highmark representatives attending, so that faculty and staff may express concerns and ask questions.

*That the LFO Council collect our concerns and submit a statement to the University Faculty Senate, Libraries Administration, and University Administration.

Further, we request:

*That the compliance deadline of November 22, 2013 be extended for at least 12 months, to enable us to learn more about the program, discuss it with University Administrators and Highmark representatives, and comply with requirements.

*That information from our doctors be accepted, without difficulty or penalty, in lieu of on-campus Biometric Screenings and online Wellness Profiles.

*That significant changes to our benefits not be unveiled during the summer, when it is more difficult to communicate through customary channels with our coworkers, Libraries Administration, University Administration, and others concerned parties.
