    Comments: The incident described above happened to a colleague and friend. It left his life shattered. I myself cannot imagine what I would do if it happened to my family or close friends. If anything can be done to prevent somebody to be hurt or killed, it is everybodies duty to make it happen. In this case the solution is there: Crossing arms protecting anybody from making a mistake that can be fatal.
