The purpose of the Petition is to remove Carolyn Jacobs-Davis as Legal Guardian of our mother Georgia M. Jacobs. Carolyn Jacobs-Davis is not performing Guardianship duties as describe in the original Petition. I, Janette Jacobs-Miller has been totally responsible for the care and welfare of Georgia M. Jacobs since this issue was decided by the Courts. Georgia M. Jacobs has not seen a Doctor in over a year. I need the legal right to continue to take care of and oversee her (Georgia M. Jacobs, Mother) health issues. I have assumed the responsibility of working in closely with Gloria Jacobs in making sure our Mother has the things and home care that is needed. But I do not have a legal right to make decisiions as overseeing and making sure she has the Health Care she need and deserves. Therefore, I am petition the Court for remove Carolyn Jacobs-Davis as Guardian. Thank you for assisting me in this matter.
