Through a consumer survey conducted last year, Carle identified the following population groups whose health care needs are not currently being met by facilities available in Coles County:
- The 4,500 people leaving Coles County for outpatient surgery and other procedures.
- The 7,400 people whose insurance does not cover providers within Coles County.
- The 80 percent of health care decision makers in the area who in a recent survey said they should have more health care choices locally.
Help Carle bring the latest in technology, research, and primary and specialty health care to Coles County.
We, the undersigned, support Carle's proposed surgery center in Coles County. By signing this petition, I give Carle Foundation Hospital permission to use my name to gain approval for a Certificate of Need with the Illinois Health Facilities Board. I also acknowledge I have not placed my name on any other petitions relating to the proposed surgery center in 2006.
