To help in reducing the confusion in cases of severe medical conditions, it is our belief that all "Living Wills" should be required by law to be in writing.It should be mandatory, just like carying a State ID at the age of 18 for ALL US citizens to have to make out a living will. Just as you would renew a driver's license, you should also be required to periodically have to renew or update their Living will, that way it reflects their most recent wishes. This would help to avoid someone claiming that as said incapacitated person got older, or saved, or had children - whatever - changed their wishes. I am sure that could still happen, but this would lessen the possibility and also make sure that people know that if they do not either update it voluntarily as their situations, feelings, opinions change, then the most recent Living Will will stand with no considerations.
In the absence of such a document, we believe the patient shall be kept alive by any and all means.
In the case of minors, such wishes shall be written by said minors' parents. With the advent of the minor's 18th birthday, the "Living Will" be written by themself.
