| 2256 | Marge Pilong | To: all health insurance providers,
The continuous glucose monitoring system (hereinafter CGMS) has been found to be an effective tool to detect impending hypoglycemia and hyperglycemia in patients before a crisis situation occurs. This technology is available today and has the ability to increase the quality of life of patients with diabetes. Every patient and physician team should have the ability to determine which persons are most likely to benefit from the use of the latest technology. I implore you to cease denying coverage for financial reasons under the guise that CGMS is "experimental". CGMS is no longer experimental and has received FDA approval after extensive research on the products.
I am signing my name, providing my email address, providing comments regarding why I support this petition and am also indicating in which state I reside. As a person who has been refused access, I am also indicating which insurance company has unfairly denied me the coverage for this life saving tool for treatment of my diabetes.
Even if I have not been directly affected by diabetes or CGMS denial, I am signing this petition as an indication of my support for patients with diabetes to be given the opportunity through insurance coverage, to improve the quality of their lives with CGMS technology.
Sincerely,
Marge Pilong | PA | Aetna |

| 2260 | Chris Hildebrand | I live in Minnesota, where the coverage for continuous glucsoe monitiring is very good. All the major insurance companies in Minnesota have a coverage poliicy. I live with diabetes and use the continuous glucose monitiring technology, and am extremely greatful to live in a state where the coverage and treatment for people with diabetes is so progressive. I am signing the petition in support of the petition, and all people who live with diabetes. Continuous glucose monitiring is the future of diabetes. It is the most significant technology with the hignest potential to improve the lives of people with diabetes since blood glucose monitiring (fiinger sticks) was introducte in the 1980.s All people with diabetes should be given the opportunity to benefit from this tremendous technology which will improve the quality of their lives on a hourly basis. | MN | My insurance company has a coverage policy. I am signing the petition in support of all the people with diabetes who do not have coverage. |
| 2261 | Kris Tiernan | I work with children with diabetes and have a son with Type 1 diabetes. He was in the CGM study. It is incredible CGM can make for anyone with type 1 diabetes. It should be a standard of care! | IA | Blue Cross Blue Shield of Iowa

| 2262 | Joan Zenner ARNP | To all insurance companies. I have several patients which I had to spend at least 10 hours per patient to get CGM. I promised my patients that I would not give up, I felt this was a device that could save their lives. It should not have to be this difficult to get this technology. How can we take care of our patients when we have to spend that much time just on one patient. Not all patients may be appropriate, but let the provider decide this. This needs to be available for all patients with Type 1 and/or hypoglycemia unawareness | IA | BC/BS |

| 2272 | Diane Hannett | My 16-yr-old daughter has T1. She has struggled with control for nearly two years. She has been denied a drivers license because of an "unfavorable medical report" reflecting her high A1C. She finished last school year on independent study because of difficulties maintaining control at school. She just wants to have the live experiences of a typical teenager and the CGMS recommended by her diabetes team would help her achieve that. But somehow our insurance company has no record of receiving the repeated letters requesting CGMS. When she was diagnosed at age 9 we told her that we would not allow diabetes to keep her from doing anything she wanted to do. Now the insurance company has turned that into a lie. | CA |

| 2298 | Anonymous | Our family fully supports the coverage of CGMs by insurance companies. Our 9-yr-old daughter has type 1 diabetes. She has been on a pump since this summer, but we are still tweaking her basal rates and I:C ratios daily. Mom is not getting much sleep since she requires nightly checks, sometimes 2-3 times per night. A CGM would certainly help us get her BG under control! | NC |
